Directions to Davis County Hospital

 Access from the South or Highway 2:

Turn north onto Highway 63, where Highway 2 & 63 intersect. Continue north on Highway 63 for .8 miles, turn left on North East Street, then take the first right on to Madison Street. Continue North on Madison Street approx. 3 blocks.

Access from the North or Highway 63:

Continue south on Highway 63, approximately one half mile from the city limits, turn right on to North East Street, then take the first right on Madison Street. Continue North on Madison Street approx. 3 blocks.

 Entrances:

Outpatient Department: Located on the south side of the hospital, visitor and patient parking is available at the entrance. Entrance Hours are 6 a.m.  – 6 p.m.

 Emergency Room: Located on the north side of the hospital. Visitor parking is available in the lot west of the entrance. The emergency entrance is open to the facility 24 hours a day.

 Medical Surgical Department: Located on the north side of the hospital, to the west of the emergency room entrance. Visitor parking is available in the lot east of the entrance. Entrance Hours are: 6 a.m. - 9 p.m.

 Long Term Care Department/Cafeteria Entrance: Located on the north side of the hospital, south and west of the emergency room entrance. Visitor parking is available in the lot to the north of the entrance. Entrance Hours are 6 a.m. - 8 p.m.
