Davis County Hospital Home and Community Health service combines public health, skilled nursing, and community health services into a single organization. The goal of staff members is to meet the total health care needs of their patients.

Our home health care staff deliver personalized services according to the physician's plan of treatment, perform follow-up and routine supervisory visits and communicate regularly with the physician.

COST

Services may be reimbursed by Medicare, Medicaid, and commercial insurance. Fees are based on the extent of services provided and each client's ability to pay. For your convenience, cost estimates are provided in advance.

HOME HEALTH CARE SERVICES

Registered Nurses

Registered nurses are available 24 hours a day, 7 days per week to make referrals, conduct home visits, and provide primary nursing services.

Homemaker Home Health Aids

 Certified homemakers provide assistance with personal grooming, exercise, ambulation as well as light housekeeping services related to patient care.

Physical Therapy

 A licensed physical therapist develops a personalized exercise program for each patient.

Public Health

The wide range of programs that are available include immunizations, communicable disease control, maternal and child services, well elderly services, and a variety of educational programs and offerings.

Meals on Wheels

This is a program to assist the elderly with meals. It is managed through our department. Volunteers deliver meals to the residents of Bloomfield at noon, seven days a week 365 days a year.
